You can add a spouse, domestic partner (mutually exclusive with a spouse), children up to age 26, and parents under the eldercare rider.

Medical, dental, vision, life insurance, retirement contribution match, and flight/travel benefits apply to all full-time employees after day 1.

Your medical insurance covers in-patient hospitalization and surgery up to 200,000/year, outpatient specialist care up to 3,000/year, maternity, and mental health. Cashless at network hospitals; reimbursement otherwise. Dependents on file share the same schedule.

1× annual base salary of employer-paid group term life is included from day one. You can buy up to 5× more during enrollment. Accidental death pays an extra 1×. Nominate beneficiaries in the portal — the most recent nomination governs payout.

Two preventive dental visits/year at 100%, basic dental at 80% and major at 50% up to 2,500/year combined, orthodontia at 50% up to a 2,000 lifetime cap. Vision: one exam/year, and frames or contacts up to 300 every 24 months.

If you're medically unable to work: STD pays 60% of base salary for up to 26 weeks after a 7-day elimination period; LTD then pays 50% of base until recovery, age 65, or the plan maximum. Coordinated with statutory schemes.

24-hour worldwide Personal Accident cover at 3× base salary. On company travel: emergency medical up to 500,000 per trip and emergency evacuation at cost. Licensed flight crew are covered for permanent loss of license at 3× base salary.
